At SFGH in San Francisco we use:

RTT w/a BBB for a floating maxilla (upper "jaw" seperated from base of skull)

Rat-a-Tat-Tat with a Base Ball Bat (ie gang fight)

What's interesting is that it happens enough that there is a term for it.
