Here is what you need, proof of a current rabies vaccination, health certificate confirming that the animal is non contagious or infectious, the health certificate has to be issued within two weeks from when you are traveling,(might be a month but I think its only good for 2 weeks. If you plan on kenneling or boarding your dog you will need proof of all other vaccinations, if not then rabies vax should be all you need.
