There are not holes in the rear doors, but it is easy (with a dremel or similar tool) to cut some.

My setup is similar to Pino's (at least the rear door speakers), except I simply got a pair of shallow speakers and installed them directly to the plastic on the lower part of the door. If you remove the trim piece, you will see there is a hole that the magnet (driver) will fit into, if it is shallow.

I built the kick panels on the rear doors with the help of a friend. I originally covered the panels in black vinyl but it did look as good as I thought it would, so I used the grey will you see in those pictures. If you're interested, I can post some pictures of the panel's build up.

I would think that if you added an Xternal fan to blow out the heat, you could mount it upside down. I have seen several models on the net.

If you mounted it OUT IN THE OPEN, whther it was inverted or not, should not affect the operation. If, however you covered the heatsink, then it would affect it (overheat, etc.). The orientation of the heatsink does not affect its operation, it would still disipate into the surrounding atmosphere. Yes, heat rises, but heatsinks operate on the theory of disipation through radiation by way of surface area.

Tweeter in NH made the custom sub box, Circut City installed the capacitor, and I installed the speakers and the deck. I tried to get the JL Audio 500/5 amp, and the Polk MOMO MM6 component 6.5" speakers on eBay (like the Alpine 7995 deck $350, and the JL 10W3V2-D2 sub $115), but I couldn't find any. I ended up buying the Polk's at Circut City on sale for $220 something.
